Chronic pain affects over 50 million Americans, and many are turning to red light therapy as a drug-free approach to relief. The evidence base is substantial — photobiomodulation has been studied as a pain treatment since the 1980s, with over 1,000 studies specifically examining its effects on pain conditions.

How Red Light Therapy Reduces Pain

RLT addresses pain through multiple pathways rather than a single mechanism:

  • Reduced inflammation — Modulates pro-inflammatory cytokines (IL-1β, TNF-α, IL-6), decreasing inflammatory pain signals
  • Enhanced tissue repair — Faster healing of the underlying tissue damage causing pain
  • Nerve signal modulation — May reduce nociceptor sensitization (pain nerve sensitivity)
  • Increased endorphin release — Some evidence for endogenous opioid system activation
  • Improved local circulation — Better oxygen and nutrient delivery promotes healing and waste removal

Conditions with Strongest Evidence

  • Osteoarthritis — Multiple meta-analyses confirm significant pain reduction for knee OA specifically
  • Tendinopathy (tendinitis) — Well-studied for Achilles tendinopathy, lateral epicondylitis (tennis elbow), and patellar tendinopathy
  • Neck pain — WHO guideline recommends laser therapy for neck pain
  • Low back pain — Emerging evidence, particularly for non-specific low back pain
  • Fibromyalgia — Smaller studies show promising results for widespread pain reduction
  • Temporomandibular (TMJ) disorders — Strong evidence from dental research
NIR Light for Deep Pain

Near-infrared (850 nm) is generally superior for deep pain because it penetrates 5–7 cm into tissue, reaching joints and deeper muscle. For surface-level pain and skin-related discomfort, red light (660 nm) may be sufficient.

Comparing Red Light Therapy to Other Pain Treatments

Red light therapy is not a replacement for medical treatment of serious conditions. However, compared to other non-pharmacological pain approaches, its evidence profile is competitive with TENS, ultrasound therapy, and massage. It has the advantage of being completely passive — no discomfort during treatment, no side effects, and usable daily at home.
